Consent For Student COVID-19 Testing In School

Dear QHST Students & Families,

Last week, we shared information via email blast and through posts to our Official QHST Website - www.queenshsofteaching.org regarding the mandatory Daily Health Screening that all QHST students and families must to complete at home each morning, prior to coming to school to engage in blended learning. Attached, please find a letter from our NYCDOE Chancellor Richard Carranza to all students and families regarding consent for students to participate in random COVID-19 testing at school.

Please complete the Consent Form below and send it with your child on the first day of On-site/In-person Learning.
